PERSONAL INFORMATION COLLECTION CONSENT

 

DreamRoads, LLC (‘DreamRoads”) collects personal information of parents/guardians and minors (some under the age of 13). For every minor, DreamRoads requires a parent/guardian’s consent to collect, use, and disclose this personal information. This Personal Information Release Form lists the various ways DreamRoads will collect, use, and disclose the parent/guardian’s and minor’s information.  

 

DreamRoads will collect the following personal information of the minor: first name, last name, age, grade, school, address, and email address. This personal information will be disclosed only to DreamRoads’ employees, staff, and contractors. DreamRoads’ Privacy Policy can be found at: www.dreamroads.com/privacy-policy

 

The parent/guardian has the right to review all personal information collected on behalf of the minor child by emailing DreamRoads at info@dreamroads.com. At any time, the parent/guardian may revoke consent and refuse further use of the minor’s personal information, including deleting the minor’s personal information by emailing DreamRoads at info@dreamroads.com. 

 

Parent/guardian releases DreamRoads, its members, managers, contractors and employees from liability for any claims (including any third party claims) in connection with this release, or the participation of any undersigned minor children.

PARENTAL/GUARDIAN AND MINOR LIKENESS AND WORK PRODUCT CONSENT

 

By signing below, the parent/guardian hereby grants permission to DreamRoads to publish any photographs, examples of student work, or video taken of the parent/guardian and/or the undersigned minor children for use in any DreamRoads printed publications, videos, or web content.

 

Parent/guardian agrees that DreamRoads may use such photographs, video, or student work of parent/guardian or the undersigned minor children, with or without the use of first names and without the use of last names, for any lawful purpose, including but not limited to such purposes as publicity, marketing, and web content.

 

Parent/guardian acknowledges that since participation in these publications, courses, videos, and web content is voluntary, neither the minor children nor parent/guardian will receive financial compensation nor shall the parent/guardian or child have any rights of ownership whatsoever.
